Agency's Report Details Events Leading To OKC Toddler's Death

OKLAHOMA CITY -- An oversight agency released a report detailing the events that led to the death of toddler Ahonesty Hicks. 

A mother on drugs, a clearly abusive boyfriend and the report from the Oklahoma Commission on Children and Youth shows DHS had the warning signs of the abuse months in advance.

The first visit to the home was 48 days before Ahonesty was killed. The first documented sign that the two children living in the home were in danger came on March 15, 2011, when Ahonesty's mother tested positive for PCP. Her two-month-old son tested positive for drugs, but was allowed to remain in the home.

On March 30, 2011 Ahonesty's mother and the infant's father, Deandre Wells, had a domestic dispute in a moving car while the kids were inside.

The report never mentions the month of April.

On the morning of May 3, 2011, Wells was giving his child a bath when the Ahonesty interrupted bath time. The report says Wells shook Ahonesty, causing brain hemorrhaging.

The toddler's grandmother, Patricia Perry, described her as "vibrant, joyful and lovable."

According to the report the usually vibrant toddler began vomiting and then became sluggish. On May 3, 2011, Ahonesty died.

It took two days for DHS to received a "near death" report and on May 11, 2011, DHS held a hearing to get Ahonesty's sibling into state custody.

Wells remains in jail without bond.  He's charged with first-degree murder and child neglect.
