Department of Health Provides Checklist To Prepare For Back To School

The state Department of Health has everything parents need to get their students ready to go back to school.

It’s almost time for kids to start thinking about their first day of school outfits, but before they step foot in class, they have to complete a few medical requirements.

Parents will need a copy of their child’s shot record, birth certificate, and current immunizations.

The state health department recommends your child also take an optional wellness exam.

“We have a whole webpage, we tried to make it really easy for parents to go on and find exactly what they need," Erica Rankin-Riley of the Oklahoma State Department of Health said. "There's frequently asked questions and hyperlinks for birth certificates or going online for those shot records. It's all on one page."

If you need a copy of a birth certificate, DHS recommends placing your order by August to avoid delays.

Contact your local provider to schedule appointments for shots, immunizations, and wellness exams.
